Hospitalized patients with acute myocardial infarction & preserved EF were assigned to receive open-label long-term beta-blocker therapy or not. Beta-blockers did not lead to a lower risk of death or MI. Full REDUCE-AMI trial results: nej.md/4akVhRx

In short, losartan is the codeine of ARBs:

- it doesn't do much on its own
- any effect is due to a metabolite
- metabolite formation is genetically determined

The result? Unpredictability and potential interactions with CYP inhibitors & inducers.

And who needs that?

No. No. Double no. Do not order troponins unless there is a question about Dx of “plaque-rupture” type ACS. Why? B/c every other cause involves treating the underlying cause (e.g, SVT, AF, anemia, fever, etc)

LVSP had worse clinical outcomes than LBBP and BVP.
But look in the methods section - do we define LVSP based on the QRSd, its reduction, or the QS pattern in V1?
Deep septal pacing was done in several patients.

38 year old presents with this ECG, defibrillated in the field, although was in that rhythm (205 bpm) for 2 hours. Normal structural heart. Thoughts? In any job there is the easy stuff, and there is the stuff that keeps it exciting.
