My first diagnosis is 🔺Acute PE✅

#ECG shows:
1. Sinus Tachycardia with HR ~120bpm
2. Kasuge - TWI in V1 and Lead 3
3. Tall R wave with STD and TWI in V1-V4 ~ RV strain pattern
4. S wave in V6- rotating vector to the right side; RV enlargement.
5. McGinn-White sign- S1Q3T3

This is great✅

Answer: OMI signs detected ‼️‼️🚑🚑

1. Subtle STE <1mm in Lead 3 and aVF with reciprocal changes aVL and Lead 1 suggests Inferior OMI
2. Maximum STD in V2-V4 suggests Posterior Wall involvement. #ECG follow-up👇

Be Careful with indigestion;

Likely P wave embedded in QRS✅with possibility of pseudo S wave in Lead 2.

OR V1 looks like p on T (but rate too fast for AT).
