This is a such a simple but salient point and Reza & Rabih Geha always stressing this is why they’re so great at what they do. I often explain my reasoning out loud to patients, invoking base rate of different issues. Pts love being able to understand why we do what we do.

William Wright Infectious Disease Fellows Network ASM TxID Early Career Network Gabe 🦊 Great case illustrating that base rate of disease is often more important than the red herring of an exposure history.

Learned to account for base rate from Rabih Geha & Reza, and my dad who always said 'common things are common.'

Thank you Ufuk for presenting such an instructive case. We totally missed it. Just incredible. Zaven Sargsyan I think you'll love this case and I'd be interested in your reflection.

Young female patient present with 10 days of the small swelling as shown in picture. There is no pain. The swelling is firm, subcutaneous. X-ray of the hand showed no bone changes. What would be the differential diagnosis? Reza Rabih Geha

After binging for 3 months on any The Clinical Problem Solvers case I possible can, there’s a high possibility that today’s new RLR Case Challenge maybe my favorite! Reza magically presents 2 different stories to Rabih Geha, who take a “temp” “change” in stride.

'Medicine bridges the gap between science and humanity, but it is through education that we traverse it, turning knowledge into healing.'

My Father's birthday was pretty recent and I think about him all the time. He was the greatest physician and teacher that I knew.

Reza this nails it! I’d add that the BioFire blood panel (BCID) also detect common resistance genes. Won’t give you actual susceptibility but is helpful in abx selection. The awesome Simon Wu and I have developed a guide for this at vaglaid.org/rapiddx.

Reza Props to you for diving into a complex, emerging, and somewhat ill-defined (at least the indications and cost-benefit analysis) part of ID.

Couple areas I can help clarify from my own novice understanding:

I cannot recommend RLR enough 🔥
You can clearly see the heart and effort poured into this project by Rabih Geha and Reza.
Wether you are in training or already practicing, you will find something tailored to your needs within RLR.

Karius, 16s rRNA, and Biofire - A brief primer from a nonexpert but hoping ID Twitter goes WILD on this thread through feedback.

Background to understand -
PCR testing vs next generation sequencing. (Shout out to GPT4 for analogy, assuming it didn't confabulate).
