Thank you Josephine Cumbo for highlighting precisely why so many doctors are deeply concerned about the role & presentation of physicians associates in the NHS.

Sometimes - as here - the public has no idea they're not seeing a doctor.

Medicolegally, so very wrong.

Physician Associates are being put into GP roles, to fill workforce gaps, but they do not have medical degrees. There have been cases where patients have been misled as to who was treating them at the GP clinic, with tragic results. bbc.co.uk/news/uk-englan…

So many concerns being raised by doctors. We definitely need more GPs - & far more training places for GPs, immediately. While trained & regulated Physician Associates may be useful in dealing with some cases, they should not be introduced haphazardly or invisibly.

Unfortunately all too common a tale.

Undifferentiated primary care is highly complex & risk filled.

Artificial bottlenecks in training, poor T&Cs for GPs & ARRS funding for PAs are resulting in patients getting poorer care.

All of this. PA wanted to prescribe Gaviscon and lanzoprozole for chest pain. I asked for a second opinion, Doctor sent me to A an E… luckily he did. Not heart related, but serious enough.

This has happened to me twice at my Drs surgery, not told I wasn’t seeing a Dr, no follow up diagnostically. I think lots of people are not seeing GPs but don’t realise.

At GP surgery today, an elderly patient with walking stick arrives at reception, and asks “who am I seeing today pls?”. Receptionist responds you are seeing “X” - only first name given to patient. “X” is listed on doctors on duty” board in the surgery reception. 1/

/2 the elderly patient then noticed a computer monitor - which allows patients to “self check in” to their appointments. He shuffles over to the monitor and starts to check in. At this point, I intervened to say the receptionist has checked him in. He seemed confused.

Having overheard the conversation between the elderly gent and receptionist, I figured out that “X” who the man was to see was not a doctor but a physician associate (PA), a non doctor doing frontline GP work at the clinic, /3

Physician Associates are currently unregulated roles, meaning there is no national standards around how they are taught, their examinations and the scope of what they do with patients.

It is concerning to me that the elderly (confused) gent in the clinic today may have believed he was seeing a doctor. I have registered my concerns with my local GP surgery about how PA roles are being presented to the public. /ends

Josephine Cumbo 2022 I saw a former paramedic at the GP, urgent apt as in a lot of hip and leg pain.
No examination, 'sciatica' and prescription for 94 Diazipam, take 3 a day. 😱😱😱
3 diazipam a day for a month. 😵‍💫

Obviously I didn't do that...

Josephine Cumbo My Mum is seen at home by surgery paramedics who are excellent, I have heard conversations between them and (also excellent) GP when unsure/worried. The problem is not the practitioners but one of openness (trust), supervision and regulation. They are NOT medically trained.

Josephine Cumbo It’s so frustrating as a patient. My pain mgt specialist wrote to my GP with a care plan and suggestions for two drugs to try. Unfortunately I had to speak to a PA. They’ve misread the report and prescribed the dose of one drug but prescribed the other.

Josephine Cumbo Surely this is the responsibility of the people who own the GP practice, which is normally the GPs. They should manage their staff as they see fit but always ask the patient if they are happy to be seen by a PA.

Josephine Cumbo several times past year i thought i was seeing a doctor, later I checked my patient records, and in all cases it was a Senior Nurse Practitioner who diagnosed and prescribed. Is this any better than the current furore over PAs?

A never event is the 'kind of mistake (medical error) that should never happen' in medicine.

There have been 12 never events linked to 143 PAs in Scotland. Such numbers are unprecedented and represent a danger to the general public

There are no shortcuts in medicine
