Our Post Discharge Frailty Support Service went live today with a digital Whiteboard. Already noticing the benefits on day 1 - more efficient, time saving, enabling a safer service and we are now paperless and saving the trees 🌳

3 solutions to the emergency care crisis from BGS President Adam Gordon:
1. Shift in attitudes - half of patients have frailty.
2. Front door frailty.
3. Investment in rehabilitation. Until we fix rehab, the revolving door will continue.

These concepts are not new & not the magic wand solution for the current pressures. The fundamental issue of workforce (with a community & frailty skill set) is lacking. Also the challenges in social care have a direct impact in supporting these models to keep people at home.
