To date the hub has been able to manage over 3,500 patients, reducing the need for an EMAS call out and providing advice, support and intervention instantly to support patients to remain safely at home.

We're delighted that our Unscheduled Care Coordination Hub has been shortlisted in the #HSJawards 🏆 This service assesses patients who've called 999 and, where appropriate, offers prompt care and support at home instead of having to go to hospital. Congrats to all involved!
