You can’t replace humans with surveillance… it’s dangerous and clearly kills... The biggest predictor of therapeutic outcome is the relationship!

Completely agree Steve, it is painful to think of the effort, the hope, and the knowledge so many people gave to this for the Government to let it go. We won’t stop calling for reform and will never give up. But this is a very sad day for mental health equality

Rajesh Mohan🌈 It’s really important to be aware of risk factors but, contrary to the view of some experts, decent clinicians don’t try to predict outcomes using these factors

We can however include them in a risk formulation and management plan

My first publication with The Mental Elf on Emanuele Osimo et al. paper on prediction models for treatment resistant psychosis

Really interesting paper to report on, and with the help of Éimear M. Foley I think the article came out great - please check it out!

Ian Davidson Samei Huda Getting It Right First Time (GIRFT) With community support, after I was sectioned I stayed out of hospital for the next 8 years

Then I was discharged by community services while unwell & - with no care plan, meds, therapy - ended up in a cycle of crisis care (acute admission, A&E, HTT)

Because d'uh

Ian Davidson Samei Huda This is indeed how services are commissioned here

I tried bringing up NICE guidance, their local policy, everything - I've tried everything

I live with severe & enduring mental illness, previously sectioned, but have no care plan, no meds, no care

I am excluded from care

🧵Tragically, smoking causes half of all deaths of people with a severe mental illness, significantly contributing to a ~20 year shorter life expectancy. So we're supporting the Government's 'Smokefree Generation' proposals & calling for additional specialist support to quit.

In medicine we seek to BALANCE harm and benefit.

Sometimes treatments have harm risks but the benefits outweigh.

Sometimes inaction causes harm and action and the best chance of reducing it.

Sometimes an alternative treatment has higher benefit but higher risk of harm.

The problem with 'first do no harm' is that it's nonsense. How could we do any medical intervention and avoid harm? We would be paralyzed into inaction and in fact our patients would not be able to benefit from any treatments.
