The reasons why they don't have access are varied. An individual child's chances can be scuttled by family dynamics, skeptical pediatricians, lack of local gender clinics, or multi-year waitlists.

The vast, vast majority of trans youth have not had puberty blockers available early enough in puberty for them to make a difference.

The vast majority don't have any hope of hormone therapy before they reach adulthood.

Something that I think is too nuanced for almost everyone in this gender-affirming care discussion is that what's being lost, more than anything, is the hope for treatment among a cohort of young trans people.
