👀 We need research into “long long covid”. In a significant proportion of people this condition goes on for YEARS.

The current strain of is apparently a virus that is not very willing to jump to humans. But we are doing everything to change it: Circulation in mink fur farms since last year, now circulation in US cows. I really fear we are soon watching a pandemic in the unfolding.

“But every dairy that I've worked with has – with the exception of one – had sick human beings at the same time they had sick cows.”

Pandemic preparedness & response plan:

Wait
Wait
Wait
Do something (but only once it’s so bad that we need refrigeration trucks)
Do nothing
Do nothing
Declare endemic
Done

The issue isn't whether anyone should panic. Of course, no one should panic. Ever. But if H5N1 becomes a genuine pandemic, our sphincters should collectively clench. Which is why it's important that people be taught the stakes *before* that eventuality.

A few thoughts on the role of pigs in the emergence of influenza A virus in mammals.

1. It is simply not the case that movement of flu viruses into non-swine mammal species requires pigs as a 'mixing vessel'.

Interesting tidbits from a press conf today on by US agencies
—NIAID-funded researchers found commercial milk that was PCR-pos for H5, but could not grow live virus from it. (Good)
—USDA & CDC admitted some farmers won't let them test.

Does anyone know if countries outside of the US are looking for in cattle and other mammals.

It seems like if not, this would be a good idea.

Grateful for CDC Dawn O'Connell U.S. FDA Dept. of Agriculture brief today on . High (non-tech) level:
- Ongoing human studies & epi w/ workers on farms, processing facilities
- Situation evolving in cows (cow-cow, cow-bird transmission)(asymptomatic infection)
- Federal order to require all

When news broke, I told David Lim, “They would have to do a lot of testing before I would drink milk from one of these farms at this point' and got a lot of pushback.

3 weeks later, U.S. FDA reported H5N1 in grocery store milk. It'll take up to weeks more to know if these

If the world needs vaccines for people, we're definitely not starting from a standing start. Lots of preparatory work has been done, especially in the US. But this would be no easy task & equitable access would likely be a huge problem. statnews.com/2024/04/24/h5n…

' US vets have heard anecdotes about more workers with these and other symptoms - such as fever, cough and lethargy - who do not want to be tested.'

Want to say this a little louder. I hope companies are making low cost, in-home, rapid tests for flu. Best would be multiplex for flu A/B, H5, C19, 3 columns or rapid home molecular tests (but very low cost). No panic, but it’s time to start development.

UK stopped collecting data on Long Covid. Switzerland NEVER started 'because cost-benefice ratio' was not satisfactory for our health minister (and his friends of course)
