An underrated bonus of GP is that we do actually manage and work-up most things across all specialties without referring.

It's intellectually satisfying.

(I know in secondary care it doesn't feel like that, but you don't see all the patients we DON'T refer.)

Interesting point here about triage - specifically that it’s reception doing the triage (which is not really triage at all).

If you’re going to use PAs in GP… should reception book directly in with them, or should they be pre-triaged by a GP?

Once was in theatre with a senior surgeon.

Me: 'Oh, did you drop the cap on that device into the field?'
Surgeon: 'No, it's in my hand. But - that's exactly what I want my assistant to do - speak up if you think I've made a mistake.'

Do medical students do this? I know of a few 'bad behaviour on social media' cases of docs + med students but they quickly faced sanctions.

Why are there seemingly lots of PA student examples of this?

Keith Siau If you are a trainee doctor or med student who feels abused, mistreated, abandoned and sidelined as well as underpaid, tired, broke from exams and underpayment, betrayed by the consultant class and royal colleges then please like and speak up. You are the best of us.

Here’s a snapshot of just 20 letters rec’d this afternoon from hospital & other NHS colleagues

Half need us to do something eg refer, order investigations, send info back

It’s not just GP time, but admin & secretarial too - when 95% staff are in hospitals

Contrast this with GP registrars - a minimum of 5 years undergrad & 2 years postgrad training - who get debrief for every case initially, should always have a named supervisor, and get a weekly tutorial (partly for discussing difficult cases).

Despite everything that is going on in the NHS and the medical community, it’s great to see people celebrate their national training number offers.

Clearly there are good people who want to work in the NHS and we really need to do everything we can to keep them.

People who are not doctors or physician associates.

When someone says “I went to medical school”, what does that make you think their qualifications are?
