This trial of ~20,000 patients from 20 North American sites was a labour of love. Our success in answering this important question is a testament to the dedication of cardiac anesthesiologists from all participating centres to set aside their personal beliefs to support science.

There were no patient-reported cases of intraoperative awareness in either arm. Given the lack of signal of arm and possible benefit, restricting intraoperative benzodiazepines should be considered!

The results of B-Free are out! What's the bottom line? In ITT restricting intraoperative benzodiazepines did not (by a hair) reduce the incidence of postoperative delirium (aOR 0.92; 95% CI 0.84-1.01; P=0.07). HOWEVER...
