Question 3. on ultra-processing. If you think a simple broiled steak is not ultra-processed, you have never been to a slaughter house. McDonald's further processing of overcooking and adding the bun is a small perturbation, however objectionable.

Question 2. on ultra-processing. The major reference on UPF (cited by Hall, e.g.) says canned food is more UPF than. frozen. The second law of thermodynamics, however, makes it clear that you need more processing to make something cold than to make it hot.

Not really important given everything going on but in answer to the ultra-processed questions:
1. unless you are on a farm, all milk is centrifuged and the fat and skimmed milk are reconstituted as required. Mixing two ingredients is not really 'ultra' but skim may win on that.
