Patient s/p Cardiofocus laser balloon ablation in 2013-nothing for 10 years! and now recurrent AF and flutter. PVs individually remained isolated but triggers firing from carina between RSPV/RIPV. The pulmonary veins are a strange beast that like to cause AF over time!

John- it’s not about challenging dogma. Many of us still remember Pappone and his push to ablate without proving PVI. Thousands of pts suffered recurrent atypical flutters. Empiric cryo may work because careful experts usually get isolation. But lack of endpoints is not progress.

Good point but I don't think will be in issue. In recently presented Manifest PFA registry with >17k pts stroke incidence was 0.12%. In ADVENT we did brain MRIs in subset of 80 pts and asymptomatic cerebral events noted in 9% of PFA pts, less than usually seen with RFA.
