In our case series of radioulnar translocation in adults.
Once reduced none required annular ligament repair.
But you need to ask yourself if I need to how am I getting to anterior attachment of annular ligament.

This injury sits in the spectrum of “radioulnar translocation” - hyperpronation.
Great example in this case report, 5 year old.

At first glance AP might be ok.
Important to know capitellum in pink, radial neck in blue.

I think in some of these hyperpronation injuries. The “bendy” ulna gets blamed for plastic deformation as the cause of the “Montegia”.
When it’s not the Ulna.
There is significant variation in the PUDA and the DUDA.

It’s a hyperpronation injury. That’s why radial head anterior and medial on top of coronoid.
Brachialis sometimes then traps it.

Looks like physeal arrest and as growth continues deformity will get worse, I would offer distraction osteogenesis of the radius.
