Over 2 years ago we set up a service to support elderly patients discharged from ED and AFAU. Today we received our STAR rating 97%!! We could not do this without our fantastic CNS and therapists in LIFT RPH our Consultant Geriatricians and our AcuteFrailtyUnitRPH colleagues #frailty ❤️❤️
Our specialist nurse Emily has received special thanks from a patient she has been following up on our Virtual Frailty Ward. We aim to keep discharged patients safe at home and prevent readmission #teamwork #continuityofcare #frailty #admissionavoidance

Fantastic joint working with Hospital Home Care @LTHtr today. I highlighted a virtual ward patient needing input. Referred and promptly assessed with equipment/care provision. Also highlighted District Nurse input needed and immediately referred. First class service keeping patients safe.
