A case example of why I check the guidewire in short and long axis views

Helps to make sure you haven’t gone all the way through the IJ into carotid or subclavian artery

If truly cannot obtain long axis then fan and follow wire down in short as far as possible

🫀You might start seeing more implantable HF monitors in your ED practice

We wrote this after we interrogated an AICD and got a “fluid index” 🤔

I liked this paper on unplanned extubations (UE)

It gives an idea on outcomes of UE & how to ⬇️

Those accidental by providers (during turns etc.) and mechanical failure (ruptured cuff, etc) place patients at highest risk for complications

Patient self extubations 🤷‍♂️

Post arrest shock is complex & heterogeneous. There’s unlikely a “ best pressor” for all

Best advice:

Try to classify shock

Cardiogenic w/ LV failure-low dose epi or norepi + dobutamine

Vasoplegic/distributive-norepi +\- vaso

Unsure? Norepi

Thanks EMPHARM-NET for the data!

Private equity acquisition of hospitals, on average, was associated with increased hospital-acquired adverse events despite a likely lower-risk pool of admitted Medicare beneficiaries, suggesting poorer quality of inpatient care.
