Q1: How can we best use data and what types of data to improve outcomes, especially with value based plans?
A1: There are two fronts. The first is the clinical front; patient population health management , chronic conditions/high clinical risks/high cost patients, refer patients.

A1: ...refer patients into care management programs, quality management (using data to address quality gaps/low measures).

A1: The second front is financial management where we use the following data; revenue/risk score to monitor costs on an ongoing basis, identify emerging trends, use data to manage provider performance.

Value-Based Payment arrangements in Medicaid aim to align payment incentives with quality and value, encouraging healthcare providers to focus on improving outcomes for Medicaid beneficiaries while controlling costs.

What is the Payer's Perspective?
-Investing in the tools and teams to leverage data analytics
-Information that may be a blind spot to providers so they may provide the best care
-Understanding why members may go out of network
-Identifying gaps
-Providing performance baselines

What is the Provider's Perspective?
-Using data to determine critical success factors.
-Identifying risk with clinical, social, financial drivers of health and using this information to be proactive
-Identifying and building interventions to manage patient load and transitions

What is the Provider's Perspective?
-Using data as quality measures for those who are insured and uninsured
-Potential for integration with specialty and hospital networks so that providers have access to the 'full picture' of a patient to better provide care.
