So many lessons, that sadly, need to be reinforced. It is important to share these experiences so others can learn. ICG, robots, etc are no replacement for meticulous techniques and adherence to principles of safe chole.

This is a must watch.

I would place pt in left lateral decubitus position and place my ports similarly to a right sided adrenalectomy. You can get up to the right diaphragm fairly easily this way.

A patient presents 3 months after laparoscopic cholecystectomy in which there were numerous spilled stones.

CT shows a phlegmon in a very superior and posterior subphrenic location (see images). The patient is moderately septic.

Great discussion! Here's what I did: needed wire to get scope in. Stones too large to extract, so crushed + flushed into duo. CBD looked clear, but on completion IOC possible stones in hepatic ducts. WWYD now??

Lap cholecystectomy in pt w h/o Roux-en-Y gastric bypass. Unfortunately cystic ductotomy avulsed when trying to milk out stones. IOC then shows CBD stones, but no obstruction. WWYD now??!?
