In 2023, Medicare beneficiaries received $882 billion more in benefits than they paid in premiums. In 1990, when today’s recipients were still in the workforce, taxpayers’ net contribution was only $99 billion ($232 billion after adjusting for inflation).

Both federal and state governments and leaders of both parties failed miserably at ensuring that only people eligible for #Medicaid were enrolled. Those who truly needed the program and taxpayers were severely harmed.

This is a key issue for understanding the health care debate over the past 15 years.

Hard to overstate how often achieving coverage (at least on paper) is treated as the end-all be-all of health policy. Other costs, trade-offs, and nuances are often minimized.

While many of these programs don’t get as much attention as other payment issues, a lot of effort (and money) goes to quality improvement efforts that don’t achieve their aims.
