Taking the time to confidently establish a diagnosis of true EGJ outflow obstruction before surgery is key - rapid drink challenge on HRM, timed barium esophagram & FLIP are important tools

State of the art lecture at on by Dr. Prakash Gyawali of WashU Gastroenterology

There are multiple possible etiologies of EGJOO diagnosis, but most important group for intervention is the achalasia-like subgroup. Correctly and confidently identifying these pts is key!

EGJOO is a heterogenous condition & many patients may have spontaneous symptom resolution. Some manometry metrics may help prognosticate, but more studies are needed

HE & DES are such confusing diagnoses! Anh Nguyen, MD shows that while many patients have spontaneous resolution, a smaller few could evolve to achalasia 🤷‍♀️ This is why I tend to treat conservatively at first, though some data on success also exists.
