The key to effective RAT is getting patients out of RAT quickly to assessment areas, SDEC, primary care or home.

If this doesn’t work, all that’s achieved is duplication and over investigation / diagnosis / treatment.

And it’s knackering.

EM is accountable for everything. When other services don’t work / stop working / seem like a “hobby” where is the accountability?

But actually I don't really hold the single organ / disease teams responsible.

Executive teams are responsible for enforcing accountability. And the evidence seems to be that they avoid doing this. Because it's too hard or unpalatable.

Until they do, we will fail.
