Stop scrolling and read this. The study too. Imposition of values is a plague in end of life care, as is repeated discussions (“maybe one more family meeting will change their mind”). It fractures trust and makes things worse. Especially for marginalized. Bravo Katie Hauschildt

1) Black and Hispanic cancer patients were more likely to receive lower doses in their final month of life than white patients.

2) Black cancer patients, especially Black men, were more likely to undergo urine drug screening by their care providers.

On pal care there were tubes and lines I didn't understand, and with that there was fear. Were we crossing a line? But dignity and beauty are also a part of death – particularly when it aligns with the patient's end of life vision. I made this art for my pal care patients 💜

“Providers didn’t all miss the day in medical school when students were taught how not to be cruel to fat people. Medical fatphobia is medicine’s status quo.”

🧵🔧 Welcome to Pipe Wrench no. 6: The Fat Issue.

It’s a big one, as is only fitting.

Sharing our new JAMA Health Forum paper. There is a shared interest in opioid access for pain and addiction. The current paradigm is not working and people w pain and/or OUD are suffering. Thank you Diana Mason for writing this piece w me. Jonas Philanthropies

I support routine family meetings to provide updates, understand patient values, etc.

I am not, however, a supporter of repeatedly meeting with family to hammer them about code status and withdrawal of support.

You build rapport in one, destroy it in the other.

I LOVE my wheelchair. Without it, I can't walk/stand for more than ten minutes (I'm missing 1/3rd of my abdominal muscles--long story). The wc has been life changing in the best way.

I LOATHE how people treat me while I use my wc. Here's some tips on how to do better, a 🧵(1/10)

We do not need more successful physicians who focus on status, publications, media appearances.

We need brave physicians who love the people enough and respect the people enough to speak out against the violence of the medical industrial complex and work towards a healed world.

So proud of my friend & educator Mollie Biewald for bravely sharing her story

'My skepticism about the need to teach empathy
started to crumble when I became the patient &
craved a bare minimum of understanding.'

One of my biggest wishes is that medical school curriculums would stop phrasing assignments and lessons with the assumption that medical students have no illnesses/disabilities/experiences as patients. That assumption alienates so many people.

It's a 'pandemic' when something bothers rich nations

It's 'endemic' when it stops bothering rich nations but continues to ravage low-income countries (e.g. TB, malaria, cholera)

How will it go with Covid-19?

NEW. One of the hardest things I've read was when the opioid epidemic was described as 'a rare case where racial bias actually protected the population being discriminated against.”

As we had the chance to share, there's nothing protective about racism.

'anything surprised you from being on the other side? Being not just the doctor, but the patient?'
Randy:'I'm not sure this surprised me, but reaffirmed that what I care most about from my providers is that they care about me as a person. It's a surprise to me how powerful it is.
