Surprising results from the FRAIL-AF trial presented today at#ESC2023.
Switching from VKA to DOAC ➡️ 69% higher rate of bleeding!
Is this because the DOAC most used was rivaroxaban?
Or do frail patients benefit from frequent INR checks & close monitoring?

Great article placing the spotlight on acute care performance from an Acute Medicine perspective. A “build it and they will come” approach to new care models is headless. Research urgently needed to better understand what works, where and for who. bmj.com/content/382/bm…

A virtual ward is only useful if it can also provide physical care. Increasing the intensity of monitoring without a credible response is likely to exacerbate the underlying problem. beds spread over miles not yards is labour intensive. Staffing is key.

A key part of the NHS's recovery plan for urgent and acute care is an increase in the number of virtual ward beds.

These have attracted media attention, but are they a panacea and does research suggest they are the solution?
