Out today: New analysis on teen overdoses in NEJM

Overdoses hit an all-time high in 2022, killing the equivalent of a high school classroom of teens each week across the US

Overdoses now the 3rd leading cause of death in kids

I care for these teens. Here's what's needed 🧵 1/

The 1,300 resident and fellow physicians at Northwestern Medicine are unionizing, because we know that, together, we can help build something better for us and our community. 💜

We're failing teens & families in addiction treatment. 🚨 In our new study (out moments ago) in JAMA, we phoned opioid addiction treatment progs for teens across the US.

Fewer than 1 in 4 actually offered standard-of-care treatment. Brief 🧵 1/ jamanetwork.com/journals/jama/…

It's National Fentanyl Awareness Day. I want to share critical info as a pediatrician & dad.

Drug overdoses now kill the equivalent to a high school classroom of teens every week across the US. About 9 in 10 are caused by fentanyl. 🧵 1/

Great piece on xylazine by the ONDCP. The numbers are striking, but, to quote Jason Beinert,“the most important number is 100% of people who use drugs are human.” Hoping the emerging threat designation brings help to people and not more stigma to drug use nejm.org/doi/full/10.10…

Thing I learned today: remember to assess for the '5 E's' of cardiac POCUS!
- Effusion
- Ejection fraction / function
- Equality (of RV/LV size)
- Exit (aortic root)
- Entrance (IVC)

Thing I learned today: the 'pseudokidney sign' is an ultrasonographic finding in intestinal intussusception where the intussuscepted segment of bowel can mimic the appearance of the kidney.

Thing I learned today: psilocybin (shrooms) and other tryptamine hallucinogens are structurally related to serotonin so serotonin syndrome is a toxicity concern, especially when used in combination with other serotonergic agents, though overall risk appears low.

Thing I learned today: ferumoxytol (IV iron) can be used off-label as an alternative to gadolinium-based MRI contrast media. Benefits include ability to use in patients with impaired kidney function, a prolonged blood pool phase, and slower intracellular uptake.

Breaking: U.S. FDA has approved Narcan for over-the-counter use! ✊🏻

I spoke in strong support of this change to their advisory panel in February. Amid ever-rising overdose deaths (including kids & teens I care for), this policy will save many, many lives.

There is no universally accepted diagnostic criteria for PFAPA but important features:
- recurrent fever (~q2-8wks, lasting 2-7d)
- flares a/w apthous ulcers, pharyngitis, or adenopthy
- normal growth/development and labs
- quick (within hours!) response to steroids

Thing I learned today: there is no gold standard imaging technique to diagnose orthopedic implant infections. After clinical and lab evaluation, MR (with metal artifact reduction), CT, US, PET, WBC scans may all be considered.
