Does necrosis predict outcome in pancreatic neuroendocrine tumors (PNETs)?

Can it be integrated with Ki67 and mitotic count to improve the grading of these tumours?

2/2✳️ Prognosis is variable
✳️The 5 year survival rate is 30%for non functioning PNETs and up to 97% for functionable resectable PNETs
✳️ 2years later FNA of recurrence in liver and pancreas showed PNET remained low grade with no evidence of transformation to high grade

This week two of my patients with stage III PNETs consented to ⁦SWOG Cancer Research Network⁩ 2104 which completely changed the conversation. We are not used to discussing adjuvant therapy for tumors and this trial is changing that. ⁦Heloisa Soares MD⁩ ⁦Syed A. Ahmad

CYSTIC PANCREATIC WELL-DIFFERENTIATED NEUROENDOCRINE TUMOR
kudos to Markie
▶️9-20% of PNETs are cystic
▶️ Body/tail > pancreatic head
▶️More favorable clinicopathologic features-single, non-functional, lower Ki-67, lower lymph node mets
▶️ Commonly mistaken for MCN

Sporadic non-functioning PNETS an 🇮🇹 multicentre analysis

✖️Pan NECs no surgical benefit
✖️Small G1 sporadic Pan-NENs no surgery needed
🔪Surgery alone not enough for G3 Pan NEN (💉!)

Multimodal approach necessary!

👏👏👏What a great initiative! I love Vinay Prasad MD MPH's lecture on PFS using PNETs.

Here is an excellent article on metastatic liver NETs, which understandably doesn't mention SBRT:

Fortunately, continues to focus on QoL and OS - not PFS! 😉
